I understand campaigns may be accepted or declined and no hours, leads, territories, assignments, or earnings are guaranteed.

I generally bear ordinary business expenses such as transportation, phone, clothing, meals, lodging, and administration, except specifically listed supplier-controlled items.

I understand that mandatory scripts, disclosures, verification, privacy, and customer-protection procedures apply to accepted campaigns.

I maintain or will maintain records appropriate for a self-employed business.

06Applicant Acknowledgments

All acknowledgments in this section are required. *

I understand compensation is commission-based and may depend on supplier acceptance, conversion, activation, supplier reporting, and supplier payment.

I understand that submitted sales do not necessarily become payable sales.

I understand that applicable chargebacks, reserves, equipment obligations, and documented offsets will be disclosed in separate agreements and compensation schedules.

I understand that no minimum hours, locations, leads, assignments, sales, compensation, or duration of engagement are guaranteed.

I understand that I may decline a campaign opportunity before accepting it, but must comply with all applicable campaign requirements after acceptance.

I understand that false or materially incomplete information may result in rejection, suspension, or termination of a contractor relationship.

I agree to promptly update Rise528 if information concerning conflicts, supplier eligibility, licenses, transportation, insurance, or compliance history materially changes.

07Conditional Background Screening Authorization

If Rise528 selects me to advance in the contractor-qualification process, I authorize Rise528 Sales Group, LLC and its designated screening provider to obtain consumer reports and/or investigative consumer reports for the limited purpose of evaluating my eligibility for independent-contractor campaigns, subject to applicable federal, state, and local law. Screening may include identity and address verification, criminal-record searches, sanctions or exclusion-list searches, and a motor-vehicle-record review when driving is required. I understand that I may be asked to provide additional identifying information through a secure process.

This conditional authorization becomes effective only if I am selected to advance and after Rise528 provides any separate disclosure, state notice, or authorization form required by law. This application is not intended to replace the standalone background-check disclosure or any additional authorization required by the Fair Credit Reporting Act or applicable state law. This authorization applies to the initial pre-engagement screening only unless I later provide separate authorization for another report.
