- Contractor applicants. Legal and preferred name, mailing address, phone number, email address, business or entity name and type, website or business profile, languages, availability and requested campaign channels, requested markets, transportation and licensing information, insurance status, sales and industry experience, referral source, your certifications and acknowledgments, and your typed electronic signature and date.
- Prospective partners. Company name, industry, contact name and title, business email, phone number, markets and channels of interest, and any message you send.
We do not ask for Social Security numbers, driver’s license numbers, financial account numbers, or other sensitive identifiers through this website. If you are selected to advance in the contractor-qualification process, any additional identifying information needed for screening will be collected separately through a secure process — not through this site.
How We Use Your Information
- To evaluate your interest in independent-contractor campaigns and communicate with you about your application.
- To respond to partner inquiries and discuss potential campaigns.
- To meet legal, regulatory, and supplier compliance obligations.
We do not sell your personal information, and we do not use it for advertising or share it with third parties for their own marketing purposes.
Background Screening
The application includes a conditional background-screening authorization. That authorization becomes effective only if you are selected to advance, and only after we provide any separate disclosure, state notice, or authorization form required by law. The application does not replace the standalone background-check disclosure or any additional authorization required by the Fair Credit Reporting Act or applicable state law. No consumer report is obtained based on the website form alone.
Electronic Communications
By submitting an application, you consent to receiving application and onboarding communications electronically at the email address and telephone number you provide. You may withdraw that consent at any time by emailing info@rise528.com, though we may then be unable to process your application.
How Information Is Handled and Shared
Form submissions are transmitted over an encrypted connection and delivered to Rise528 through a third-party form-processing service, which handles the data on our behalf and is not permitted to use it for its own purposes. We may also share information with:
- Screening providers we designate, if and when you are selected to advance and have provided any legally required separate authorization.
- Suppliers or providers whose campaigns you are being considered for, where their program requires it.
- Professional advisors, or where required by law, subpoena, or lawful government request.
Retention
We retain application and inquiry information for as long as needed to evaluate your submission, to manage any resulting contractor relationship, and to satisfy legal, tax, and recordkeeping obligations. You may ask us to delete your application by emailing info@rise528.com; we will honor the request except where we are required to retain records.
Your Choices
You may request access to, correction of, or deletion of the information you have submitted by contacting us at info@rise528.com. Depending on where you live, you may have additional rights under state privacy law; we will honor those rights as required.
Cookies and Analytics
This website does not set advertising or tracking cookies and does not run third-party analytics. Fonts are loaded from Google Fonts, which receives your IP address as part of serving that request.
Children
This website is not directed to children. Contractor applicants must certify that they are at least 18 years old.
